A successful transition into the real world is critical for these children and teens. Although we strive to give our children the time, space, and support to heal and grow we also know we must prepare them for the real world which is fast-paced and not as supportive. Whether from the residential program or a “day” student, each child will have a transition plan and support team to help them prepare in advance and create a “bridge” with parents and teachers in their real world. There will also be follow-up calls and contact with family, school personnel and the children to support their successful transition.

Transition goals will be written and implemented upon admission. The admission team will also identify eight transitional “anchors” to start establishing relationships for support. The Hope Ranch will be committed to long-term comprehensive care of children, including post-care support for young adults in transition from state custody and its future alumni of all ages.
