A successful transition into the world outside of Hope Ranch is critical for these children and teens. Giving them the time, space, and support required for healing and growth, while also anchoring them in the local community, will prepare them for the fast-paced world that is not always so supportive.
Transition Goals
Immediately upon admission to Hope Ranch, each child’s transition goals are established and documented. Establishing these goals when a child enters the program allows staff to:
Tailor education and counselling to help students reach their goals
Determine community “anchors” and help students establish supportive relationships
Implement a transition plan and create a support team to ensure success.
Continued Support
Hope Ranch is committed to long-term comprehensive care of children, including post-care support for young adults in transition from state custody and future alumni of all ages.
After a child transitions out of Hope Ranch’s care, follow-up calls and contact with the children, families, and school personnel will allow for relational consistency and continued support for children as they navigate through adulthood.
